Post-Acute Care

The Compass Programme

Bridging the gap between residential treatment and independent living — with the support you need to build a sustainable recovery.

The transition from residential treatment back to everyday life is one of the most critical — and most vulnerable — moments in the recovery journey. The Compass Programme was designed specifically to support patients through this transition, providing the structure, therapeutic support, and practical guidance needed to consolidate the gains made in treatment and build a genuinely sustainable recovery.

The Compass Programme is a unique offering. Very few private providers offer this level of post-acute continuity, and it is one of the most important ways in which Cardinal Clinic's model of care differs from that of a standard private clinic.

What the Programme Includes

Structured Transition Planning

A detailed, personalised plan developed with your clinical team before you leave residential care, covering every aspect of your return to daily life.

Ongoing Individual Therapy

Continued individual psychotherapy with your therapist, maintaining the therapeutic relationship built during treatment.

Group Therapy & Peer Support

Regular group sessions with others at a similar stage of recovery, providing community, accountability, and shared learning.

Behavioural Health Coaching

Practical, goal-focused coaching to help you navigate the challenges of daily life — work, relationships, finances, and routine.

Relapse Prevention

A structured relapse prevention programme, including identification of triggers, development of coping strategies, and crisis planning.

Family Support

Dedicated support for family members, helping them to understand the recovery process and play a constructive role in your ongoing wellbeing.

Alumni Network

Access to Cardinal Clinic's alumni community — a network of individuals who have completed treatment and are committed to long-term recovery.

Who is the Compass Programme for?

The Compass Programme is designed for patients who have completed a period of residential or intensive day patient treatment and are ready to transition back to independent living — but who would benefit from continued structured support.

It is also available to patients referred from other residential providers who are looking for a high-quality post-acute programme to support their ongoing recovery.

"

The work done in residential treatment is essential — but it is what happens in the months afterwards that determines whether that work lasts. The Compass Programme exists to make sure it does.
